By accessing this website, submitting a request, scheduling an appointment, or using our online services, you agree to these Terms & Conditions.
1. Our Services
Lab-to-You Services provides mobile phlebotomy and specimen collection services for individuals, healthcare providers, facilities, employers, and other organizations in St. Charles, St. Louis, and surrounding Missouri communities.
Services may include mobile blood collection, specialty laboratory kit collection, standing-order collections, specimen pick-up and delivery, and contract mobile phlebotomy services.
Availability of individual services may vary based on location, laboratory requirements, provider orders, specimen requirements, scheduling availability, and other factors.
2. Mobile Phlebotomy Is Not Medical Diagnosis or Treatment
Lab-to-You Services provides specimen collection and related support services.
Unless expressly stated otherwise, Lab-to-You Services does not diagnose medical conditions, prescribe medications, interpret laboratory results, establish treatment plans, or replace the medical advice of your physician or other qualified healthcare provider.
Questions regarding laboratory results, diagnoses, treatment, medications, fasting requirements, or the medical necessity of testing should be directed to your healthcare provider or ordering provider.
3. Laboratory Orders and Requisitions
Certain collection services may require a valid laboratory order or requisition from an authorized healthcare provider or laboratory.
You are responsible for providing complete and accurate documentation when required.
Lab-to-You Services reserves the right to delay, decline, or reschedule a collection when necessary documentation is unavailable, incomplete, inconsistent, or does not provide sufficient collection instructions.
4. Specialty Laboratory Kits
Specialty laboratory kits may have specific collection, processing, timing, handling, shipping, or preparation requirements.
Submission of a specialty-kit request, requisition, or collection instructions does not guarantee that Lab-to-You Services can perform the requested collection.
Specialty-kit requests may be reviewed before an appointment is accepted or confirmed.
Lab-to-You Services reserves the right to decline a specialty collection when the collection requirements cannot reasonably or safely be accommodated.
5. Appointments and Scheduling
Appointments are subject to availability and service-area limitations.
When scheduling, you agree to provide accurate information, including your name, contact information, collection location, requested service, and any other information reasonably necessary to coordinate your appointment.
Appointment confirmation may be provided electronically by email, text message, or another designated communication method.
You are responsible for ensuring that the collection location is safe and reasonably accessible to the mobile phlebotomy professional.
6. Pricing and Travel Fees
Mobile collection services currently start at $75.
The amount displayed or quoted for a service may vary depending on factors such as:

Additional travel fees may apply based on location.
Unless specifically stated otherwise, Lab-to-You Services' collection fees are for the mobile collection service itself and do not necessarily include laboratory testing, laboratory processing, physician services, or other third-party charges.
Corporate, facility, and contract services are priced separately based on the organization's needs.
7. Payment
Payment may be required in full at the time of booking unless another billing arrangement has been approved by Lab-to-You Services.
Healthcare facilities, businesses, and contract partners may be eligible for invoicing or other approved billing arrangements.
Where available, online payments are processed through the payment methods and payment processors made available through the website.
Lab-to-You Services reserves the right to modify prices and payment requirements. Any applicable charges will be disclosed before the relevant transaction is completed whenever reasonably practicable.
8. HSA and FSA Payments
Where HSA or FSA payment methods are accepted, eligibility for reimbursement or payment is determined by your individual plan administrator or benefits provider.
Lab-to-You Services does not guarantee that any particular service will qualify as an eligible HSA or FSA expense.
You are responsible for confirming eligibility with your plan administrator.
9. Cancellations, Rescheduling, and Refunds
Appointments may be subject to cancellation, rescheduling, no-show, or refund requirements established by Lab-to-You Services.
Any applicable cancellation or refund policy presented during the booking or checkout process forms part of these Terms &
Conditions.
Clients are encouraged to provide as much notice as possible when an appointment needs to be changed.
Lab-to-You Services may also need to reschedule an appointment because of circumstances including severe weather, unsafe conditions, staff availability, transportation issues, incomplete documentation, laboratory requirements, or circumstances outside our reasonable control.
10. Preparing for Your Collection
You are responsible for following any preparation instructions provided by your physician, ordering provider, laboratory, or specialty-kit provider.
These instructions may include requirements related to fasting, hydration, medication timing, collection timing, or other preparation.
Lab-to-You Services does not independently modify instructions provided by your healthcare provider or laboratory.
Failure to follow required preparation instructions may affect whether a specimen can be collected or accepted by the laboratory.
11. Specimen Handling and Laboratory Processing
Lab-to-You Services will use reasonable care in collecting, preparing, handling, and transporting specimens within the scope of the services being provided.
Once a specimen is transferred to an independent laboratory, courier, healthcare provider, shipping carrier, or other third party, subsequent testing, processing, reporting, and handling may be subject to that party's procedures and policies.
Lab-to-You Services does not control and cannot guarantee laboratory turnaround times.
12. Laboratory Results
Laboratory testing and interpretation of results may be performed by independent laboratories or healthcare providers.
Lab-to-You Services does not guarantee when laboratory results will become available and does not guarantee a particular testing outcome.
Questions concerning your results or their medical significance should be directed to the appropriate laboratory, ordering provider, or healthcare professional.
13. Health and Personal Information
Certain services may require the submission of personal information, laboratory orders, requisitions, collection instructions, or protected health information.
Information submitted through the website will be handled in accordance with our Privacy Policy and applicable law.
When HIPAA applies to Lab-to-You Services' activities, protected health information will be handled in accordance with applicable HIPAA requirements.
Lab-to-You Services may utilize website technologies designed to support appropriate protection of health information. Users should only submit health information through forms or communication methods specifically designated for that purpose.
Wix currently provides PHI Protection for eligible sites and a Business Associate Agreement for businesses using its HIPAA compliance features. Wix also cautions that site owners are responsible for their configuration, applications, and handling of sensitive information.
14. Emergency Medical Situations
Lab-to-You Services is not an emergency medical service.

15. Healthcare Organizations and Contract Services
Healthcare providers, facilities, employers, research organizations, and other businesses may request contract or recurring collection services.
Submitting a contract-service request does not create a contractual relationship or guarantee service.
Specific pricing, scheduling, responsibilities, billing arrangements, service volumes, and other terms may be established through a separate written agreement between Lab-to-You Services and the organization.
If a separate written service agreement conflicts with these website Terms & Conditions, the applicable provisions of the separate agreement will control with respect to the contracted services.
